Ran It Twice

Your agent compacts old turns into a rolling summary once context passes a threshold. Right after compaction it re-runs a database migration it had already run earlier in the same session. What went wrong?

The context window overflowed and the provider silently truncated the system prompt holding the instructions

The record of completed actions existed only in context and compaction removed it; that state belongs in a durable task ledger

The summary was inserted after the newest user message, so the transcript ordering confused the model about what had already run

The embedding model changed between turns, so the stored migration memory no longer matched the query that would have retrieved it
